Definition
1. The benefit of getting a new card in the event of a stolen card entails UL providing a new card of the same kind as the card which was lost.
Conditions
2. This benefit applies only if the UL-card was purchased online on the UL website.
3. Only the person who ordered the UL card is entitled to this privilege.
The authorized user is normally the person who bought the UL card for him or herself. Alternatively, the authorized user may be the person who received a card which was purchased for him or her by another person (this is often the case when a parent/guardian buys a card for a minor or when an employer buys a card for an employee.)
Both the person who bought the UL card for himself and the person entitled to use it, e.g. a minor, are hereinafter referred to as the authorized user.
4. A new UL card can be obtained only after a complete written declaration has been submitted in accordance with UL's form (see below) along with a copy of police report.
The declaration of loss and attached copy of the police report must be sent to UL as soon as possible, but no later than ten days after discovery of the stolen card. The authorized user will usually receive a decision from UL within five working days.
The replacement card will be sent to the address indicated by the authorized user in the declaration of loss.
5. UL will nit provide refunds for cards issued to replace lost UL Cards.
Limitations
6. The benefit does not apply to:
- Stolen cards which have been handed over to, or used by, persons other than the purchaser.
- loss of a UL card due to negligence of client, for example the theft of a UL card because it was stored in unguarded clothes, rooms or cars.
- declarations of stolen UL cards sent later than ten days after the loss was discovered (or should have discovered)
- other costs or damages incurred as a result of the loss.
- stolen UL cards when less than 20 days remain on the card's period of validity. IMPORTANT! This does not apply to 10-trip cards or the youth card.
- More than one loss per authorized user per year. The authorized user must wait 12 months to invoke the benefit of entitlement to card replacement.
- Situations where UL would otherwise be considered to have reasonable grounds to refuse a customer to receive this benefit. For example, UL might refuse a customer to invoke the benefit in the event of reasonable suspicion of abuse, such as multiple losses reported by the same person or household.
Misplaced UL cards will not be replaced.
